Infrastructure to Support Knowledge Base
The main activity is to create an electronic data system that can be used to improve clinical care delivered to diverse urban populations, to identify populations requiring targeted interventions to improve and sustain their health, and to track over time the health care utilization, cost and health status of Minnesota's uninsured and medical assistance patients.
